Youth Hubs Operations

Provide low barrier access to a range of core services and secondary supports that meet the needs of youth;
Include non-traditional hours of operation, to ensure youth can access services and supports when they need them;
Have an upstream focus on prevention and early intervention services, supports and resources;
Take a holistic approach to youth wellness;
Offer walk-in, scheduled and virtual programming and services that are responsive to the identified needs of youth;
Involve youth as key partners in a meaningful and ongoing way as it applies to their care and in the development and ongoing operation of HOMEBASE hubs;
Work in partnership with the HOMEBASE backbone team to achieve a shared vision and shared outcomes related to building a Learning Health System;
Offer comprehensive, effective, high quality and evidence-informed programming and services;
Ensure that a standard universal consent policy and form be developed to ensure seamless and ethical information sharing;
Provide a safe, inclusive, and welcoming space for youth in the community;
Utilize highly trained and qualified service providers and staff; and
Deliver ongoing quality improvement through data, monitoring, research, and evaluation.

Overview of Services

HOMEBASE aims to coordinate, streamline, and build on existing resources in each community to provide youth, caregivers, and families services at no cost. The HOMEBASE model co‑locates and integrates services for all youth. Services will be aligned with regional Saskatchewan Health Authority services to create a pathway of care.

Mental health and addiction services

Walk-in mental health and addiction services.
Information and resources that support youth and caregiver understanding and overall wellness.

Physical health services

Primary care services provided by a licensed physician or nurse practitioner.
Access to sexual health and health promotion services and resources.

Education, employment, and training supports

Spanning the continuum of elementary/middle years/high school, post-secondary or skills training and employment supports.

Community and social services

May include but is not limited to housing supports, legal aid services, financial supports, etc.

Peer supports

Use their lived experience to help and support others.

Cultural and traditional supports

Staff, services and supports will be culturally informed, trauma-informed, anti-oppressive and recognize the role racism plays in achieving wellness.
Services and supports will be guided by traditional ways of knowing, following the principles of cultural humility.

HOMEBASE operates hubs serving young people on Treaty 4 and 6 Territories, the traditional lands of the Nehiyaw/Cree, Saulteax, Dakota, Lakota, Nakota, Stoney and homeland of the Métis. As we move forward with this work, we engage with each unique community to ensure the needs of all young people are met in a culturally responsive way.
